A Clinical Case Manager coordinates medical care and provides advocacy to help people get the best and most affordable care. Helps patients navigate insurance and hospital bureaucracies and access information. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment and travel to different doctors' offices, hospitals or a patient's home.

Our Brands Responsibilities:
Provide individual, group, and family training to children, adolescents and/or adults in assigned region/counties ( For generalist year clinicians) . Provide individual, group, and family counseling to children, adolescents and/or adults in assigned region/counties ( For clinical year MSW interns, or other master/doctorate level clinicians ). Collaborate with other Georgia HOPE staff and community partners to ensure effective treatment, service provision and case collaboration. Document case notes in online electronic medical record system (CareLogic). Attend regular team meetings and clinical supervision. Attend agency trainings, as needed.
Qualifications:
Currently enrolled in a Master's degree program in Counseling, Social Work, or a related mental health field. Proficiency in basic computer skills, including internet research and Microsoft Word applications. Proven ability to proficiently navigate the CareLogic online electronic medical record system. Ability to work independently and manage time efficiently Ability to function as a member for a multi-disciplinary treatment team and work cooperatively with other agency employees Ability to conduct oneself professionally and ethically with clients, co-workers and community partners Provide individual, group, and family training to children, adolescents and/or adults in assigned region/counties (For generalist year clinicians).
