8am - 4:30pm Summary/Objective The Case Manager provides services to inpatient, observation, emergency department, swing be and resident home patients and is a patient advocate. This role assesses patients on admission for discharge needs and planning including home services, DME, short term and long term placement, financial assistance, emergency assistance, and psychiatric placement. Possess current working knowledge of third party payer guidelines and understanding of inpatient criteria. Competencies

The hospital operates rural health clinics in Watseka, IL, Gilman, IL, Milford, IL, and Kentland, IN. The hospital also operates a skilled nursing facility, Iroquois Resident Home, which is attached to the hospital property.
