POSITION
Targeted Case ManagerLOCATION
Elizabethtown, KYQUALIFICATIONS
Bachelor's degree in human services (or a related field) Targeted case management (TCM) certification Completion of case management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) within six months of employment Completion of recertification requirements (KBHDID) every three years Experience working as a TCM preferredRESPONSIBILITIES AND TRAINING
Coordinate services for patients with severe emotional disabilities, mental illnesses, and substance use disorders Perform patient assessments to determine strengths and needs (medical, social, functional, educational) Perform periodic reassessments Arrange for transportation and delivery of services Facilitate referrals regularly communicate with patients about their health, well-being, and appointments Provide documentation (care plans, forms, reports, narratives) communicate with the Rosenwald outpatient treatment team Participate in weekly treatment team meetings Other duties as assigned Be prepared for crisis intervention, first aid, CPR (training provided)SHIFT:
Monday-Friday 8am-5pm est.BENEFITS
