Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for inpatients recovering from complex conditions. You will assess patient needs, develop individualized discharge and care plans, collaborate with physicians, therapists, and nurses, and secure services and equipment to support safe transitions home or to the next level of care.
