Job Description

Job Overview We are seeking a dedicated and organized Case Manager to oversee members enrolled in our GAFC program in North Shore area. The Case Manager is responsible for conducting assessments, care planning, compliance monitoring, and ongoing coordination of services in accordance with MassHealth regulations. Duties Conduct initial assessments to evaluate clients' needs and determine appropriate services. Perform bi-monthly in-home visits to monitor client status and provide ongoing support. Coordinate services related to Section 8 housing, Medicare, workforce development programs, and other community resources. Assist and support members in scheduling medical appointments and arranging transportation. Collaborate with healthcare providers, social service agencies, and community organizations to ensure comprehensive care. Maintain accurate and detailed documentation of client interactions, progress notes, and service plans in compliance with agency standards and regulations. Advocate for clients to facilitate access to housing assistance, healthcare benefits, employment opportunities, and other essential resources. Monitor client progress. Requirements A Bachelor's degree in Social Work, Nursing, Human Services, Psychology, counseling or a related fields. At least 2 years of experience working with elders or adults with disabilities. Certification or licensure in social work or counseling is a plus but not mandatory. Or a Bachelor's degree and 2 years of clinical experience it the care of elders or people with disabilities. Excellent communication skills with the ability to engage diverse populations effectively. Ability to handle sensitive situations with professionalism and empathy. Ability to work independently as well as part of a multidisciplinary team committed to client success.
