Our person centered approach helps clients strengthen daily living skills, work toward recovery goals, access community resources, and participate more fully in their communities. We work with clients, families, advocates, therapists, and other providers to support consistent, coordinated services. We are seeking a compassionate and detail oriented Certified Psychiatric Rehabilitation Practitioner (CPRP) who understands that effective rehabilitation depends on both meaningful client support and reliable daily follow-through. The person in this role will help assess rehabilitation needs, develop and monitor plans, guide staff, review documentation, and address service barriers promptly. What you will do Rehabilitation assessment and planning Review referrals, available clinical information, client histories, and other records needed to understand each client's rehabilitation needs. Meet with clients, and with families or guardians when appropriate, to identify strengths, preferences, functional challenges, and recovery goals. Complete assigned psychiatric rehabilitation assessments and contribute to individualized rehabilitation plans. Write practical, measurable goals related to areas such as daily living, social skills, wellness, education, employment, family relationships, and community participation. Review client progress, identify when a plan needs to change, and complete updates within required timeframes. Use trauma informed, culturally responsive, age appropriate, and person centered practices. Daily program and staff support Work with supervisors and PRP advocates to ensure services reflect each client's rehabilitation plan. Provide staff with guidance on rehabilitation goals, appropriate skill building activities, client engagement, and clear progress documentation. Review caseload concerns, missed contacts, barriers to participation, and clients who may need additional follow-up. Help identify gaps in staff documentation and communicate corrections or training needs to the appropriate supervisor. Participate in supervision, case consultations, team meetings, training, and quality improvement activities. Keep management informed of issues that may affect client care, documentation, authorization, or program compliance. Care coordination and follow-up Communicate with therapists, prescribers, referral sources, community providers, vendors, and families or guardians when appropriate and properly authorized. Help coordinate access to resources that support a client's rehabilitation goals, including housing, benefits, education, employment, and other community services. Track and escalate barriers such as missed therapy appointments, incomplete therapy verification, insurance changes, difficulty contacting a client, or interruptions in services. Collaborate with intake and management staff to help resolve missing information that delays appropriate services or authorization. Support continuity of care when a client's needs change or a referral to another qualified provider is needed. Documentation and compliance Complete assigned assessments, rehabilitation plans, updates, case communications, and other records accurately and on time in ICANotes. Review client charts for required information, signatures, service documentation, and consistency between documented services and rehabilitation goals. Follow Helping Hands PRP, Inc.'s current documentation deadlines and promptly report barriers that may prevent timely completion. Assist supervisors with preparation for internal chart reviews, audits, and other quality assurance activities. Maintain client confidentiality and handle protected health information in accordance with HIPAA and company policy. Follow applicable Maryland program requirements, payer requirements, ethical standards, and organizational procedures. Client safety and professional judgment Recognize changes in a client's presentation or circumstances that may require prompt attention. Respond to urgent concerns within company procedures, notify the appropriate qualified clinical staff or supervisor, and help coordinate emergency assistance when needed. Document concerns, notifications, and follow-up accurately. Maintain appropriate professional boundaries and treat clients, families, and coworkers with respect. A typical workday Daily priorities may include reviewing client charts and outstanding documentation; completing rehabilitation assessments or plan updates; consulting with advocates and supervisors about client progress; contacting authorized providers or referral sources to address service barriers; following up on missing records; and documenting actions in ICANotes. Priorities will vary with client needs, deadlines, and direction from program leadership. Required qualifications Current Certified Psychiatric Rehabilitation Practitioner (CPRP) certification. Experience in psychiatric rehabilitation, mental health, behavioral health, or a closely related service setting. Understanding of recovery oriented services, rehabilitation planning, and practical skill building. Strong assessment, documentation, organization, and follow-up skills. Ability to communicate professionally with clients, families, staff, management, and outside providers. Ability to manage competing deadlines, recognize problems early, and escalate concerns appropriately. Commitment to confidentiality, ethical conduct, and accurate recordkeeping. Ability to work in person at our office Monday through Friday, 10:00 a.m.-5:00 p.m. Preferred qualifications Experience with Maryland psychiatric rehabilitation programs. Experience supporting children, adolescents, families, or adults with complex behavioral health needs. Familiarity with developmental disabilities, autism spectrum disorder, trauma informed practices, or co-occurring needs. Experience reviewing charts, guiding staff, or participating in quality assurance. Familiarity with electronic health records, especially ICANotes. Knowledge of community resources in Baltimore City and surrounding areas. What success looks like A successful CPRP keeps rehabilitation plans current and useful, documents work on time, gives staff clear guidance, follows through on client barriers, and communicates concerns early. Most importantly, the CPRP helps ensure that clients receive organized, respectful support tied to their individual recovery goals.

This is a psychiatric rehabilitation position. CPRP certification is a professional certification; it is not a Maryland clinical therapy license. Psychotherapy, clinical diagnosis, and other services requiring a separate professional license are performed only by appropriately licensed and authorized clinicians.
