County About the Role Are you a problem-solver with a passion for advocacy? As a Targeted Case Manager, you won't just be managing a caseload—you'll be the bridge between our participants and the life-changing resources they need. You will work on the front lines, helping individuals with mental illness, navigate healthcare, housing, and social systems to achieve independence and wellness.

What You'll Do Assessment & Planning:

Conduct comprehensive assessments of participants needs and develop individualized, person-centered service plans.

Advocacy:

Act as a fierce advocate for participant's rights and access to state, federal, and local benefits.

Coordination:

Link clients with medical, social, and educational providers; monitor service delivery to ensure it meets the participant's evolving goals.

Crisis Intervention:

Provide immediate support and resource linkage during critical situations.

Documentation:

Maintain high-quality, clinical documentation in compliance with Medicaid standards.

Who You Are Qualified:

You may hold a Bachelor's degree in Social Work, Psychology, Sociology, or a related human services field.

Organized:

You can juggle multiple priorities and deadlines without breaking a sweat.

Empathetic:

You possess a deep understanding of the challenges faced by vulnerable populations.

Mobile:
