Summary:
The Clinical Case Manager I - ACT role provides mental health rehabilitation services, supporting adults and families to reduce hospitalization and crisis episodes. This role delivers essential services, primarily in the clients living situation, including crisis intervention, case management, individual coaching, monitoring for medication adherence and skills training. Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education:
Bachelor's Degree from an accredited institution with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Corrections, Rehabilitation, or closely related field required.
License/Certifications:
MHRT-C Certification required if not clinically licensed. Eligibility for LSW license preferred. Valid Driver's license and appropriate level of automobile liability insurance required.
Experience:
Five years of experience working with a broad spectrum of mentally ill clients preferred. One year of experience in the same area of education specialization preferred.
