Job Description Summary Position Summary The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience. We are seeking a skilled and patient-focused Case Manager (RN) to join our growing team.
