Responsibilities Conduct comprehensive patient assessments to identify needs related to medical, social, behavioral health, and discharge planning. Develop individualized care plans that align with clinical guidelines and patient preferences, ensuring optimal health outcomes. Collaborate with healthcare providers, social workers, and community resources to coordinate services and support continuity of care. Manage utilization review and utilization management processes to ensure appropriate level of care and resource allocation. Document all interactions accurately within EMR (Electronic Medical Record) or EHR (Electronic Health Record) systems while maintaining HIPAA compliance. Assist patients with intake procedures, social services referrals, and discharge instructions tailored to their cultural backgrounds. Provide client communication that is clear, empathetic, and informative to foster trust and engagement throughout the care journey. Requirements Proven experience in critical care settings such as ICU (Intensive Care Unit), emergency medicine, or hospital medicine. Strong background in nursing or social work with knowledge of medical terminology and healthcare workflows. Familiarity with CPT coding, ICD-9/10 coding systems for accurate documentation and billing purposes. Experience working within managed care environments, health insurance processes, or utilization review is highly desirable. Proficiency in Microsoft Office applications along with EMR/EHR systems for documentation and communication. Knowledge of discharge planning strategies for diverse populations including pediatrics, behavioral health patients, and those in hospice or trauma centers. Excellent client communication skills with the ability to work effectively with individuals from various cultural backgrounds; bilingual abilities are a plus. Ability to handle complex cases involving Level I or Level II trauma centers or pediatric intensive care units (PICU).
