8:30 - 5:00
Qualifications:
2 years home health experience Kinnser/OASIS experience (Preferred) Strong analytical/critical thinking skills Ability to work in a fast pace environment
Duties:
1) Review/QA therapy documentation for clinical accuracy and compliance, manage assigned therapy caseload from admission through discharge, and maintain accountability for case progression and outcomes. 2) Ensure compliance with Medicare Conditions of Participation, state/federal regulations, and agency policies. 3) Supervise and provide feedback to clinicians regarding documentation and care issues, ensuring care plans are delivered appropriately. 4) Communicate effectively with field clinicians, nursing, intake, and other office staff, while also assisting them as needed. 5) Ability to coordinate effective and appropriate labor utilization in a fast-paced regulated healthcare environment. 6) Serve as a primary point of contact for therapy-related issues and address patient concerns related to therapy care, scheduling, and services. 7) Communicate with the physicians' office to ensure that physician orders are obtained, updated, and documented appropriately. 8) Attends and actively participates in case conferences, interdisciplinary meetings, and in-services.
