HOMECARE DOCUMENT SPECIALIST/PORTAL MANAGER JOB SUMMARY
Monitor assigned portals for new referrals. Collect documents for Face to Face requirements from referral information and/or referral sources including online EMR portals. Review collected documents to ensure they meet the requirement for Review Choice Demonstration. Coordinate with Homecare Liaisons to collect required documents. Ensures documents are saved and uploaded to the Liberty EMR in a timely manner.
JOB REQUIREMENTS
Must be a high school graduate or equivalent. Two years experience working with home health, hospitals, physicians, HMOs, hospice, outpatient services or social services concerning public relations or office procedures, preferred Must possess a working knowledge of home health Medicare services, specifically to include Face to Face requirements. Demonstrate strong communication, organizational, and time management skills. Demonstrate attention to detail and problem-solving skills. Must have basic computer skills in use of Microsoft products and the ability to use a keyboard and mouse. Able to utilize electronic record portals to access and retrieve medical record information. Utilize phone systems including TEAMS phones with excellent telephone etiquette. Must interact professionally with all customers both internal and external.
