JOB SUMMARY
This position works closely with the VP of Clinical Services to ensure managed facilities are in compliance with resident assessments and staff education requirements, in support of the mission and values of PARUAN. This position also assists with implementation of new policies and procedures as regulations change and other leadership duties as needed.
QUALIFICATIONS
Must be registered and licensed with the State of Minnesota as a RN and must maintain a current and unrestricted licensure. Have some experience with Assisted Living and or Home Care and Assessment Process. Must have a valid driver's license, if driving is required for the role. At least 3 years' experience in related field.
ESSENTIAL JOB FUNCTIONS AND TASKS
Complete needed assessments under rule 144G, including completing essential paperwork and complying with state regulations: Works to assure managed facilities remain in compliance with Assisted Living regulations, including maintaining current knowledge of Assisted Living licensure requirements and being available for licensure surveys as needed. Works with VP of Clinical Services to keep forms updated, including policies, procedures, Bill of Rights, and related assisted living forms and manuals. Maintains an up-to-date knowledge of current trends regarding Assisted Living facilities. Reports incidents of suspected resident neglect and/or abuse to the facility Clinical Nurse Supervisor, Licensed Assisted Living Director, VP of Clinical Services and/or appropriate county officials. Works to maintain privacy and confidentiality of resident, resident's records, and resident's living environment. Prepares, records, submits, and maintains accurate and timely resident documentation, including correspondence, billing information, assessment paperwork, and required resident records. Communicates changes in resident needs, conditions, or care requirements (including medication updates) to the appropriate care team members (facility nursing staff, physicians, and county caseworkers as applicable) and recommends alternative placement if necessary. Implements new service agreements with residents as needed and ensures care/service plans are reviewed, updated, and current following each completed assessment, keeping facility staff informed. Facilitates coordination of resident services with other service providers as needed. Supervision of nursing staff and unlicensed personnel: Supervises, orients, and competency-tests unlicensed staff on assisted living policies and procedures, incorporating observation-based feedback as needed. Assists with health and medication training for new employees and provides continued education for staff, documenting training to ensure compliance with Assisted Living regulations.
Team Collaboration:
Develops and maintains a positive working relationship with PARUAN staff and other facility staff. Articulates information regarding residents to the team. Attends and contributes to staff meetings, committees, and related team activities. Maintains professional conduct and ethical standards consistent with facility policy in interactions with residents, families, and staff. Adjusts to changes in scheduling, resident needs, or facility operations as required. Follows facility safety protocols during emergencies, including fire drills and other emergency procedures. Reports injuries for self, staff, residents, or visitors immediately to the Assisted Living Director. Keeps the Licensed Assisted Living Director informed and advised of needs and problems. Other duties as assigned.
OTHER SPECIALIZED KNOWLEDGE AND ABILITIES
Possesses sensitivity and aptitude for working with the elderly. Possesses skills to communicate effectively with supervised staff, residents, family members, the Clinical Nurse Supervisor, and the Licensed Assisted Living Director. Possesses leadership/supervisory skills. Able to prioritize and organize work effectively and efficiently.

$93,600.00 - $104,000.00 per year
