Responsibilities Lead quality improvement initiatives utilizing HEDIS measures and quality performance metrics. Conduct chart reviews, clinical audits, and population health data analysis to identify trends, gaps in care, and opportunities for improvement. Develop and implement corrective action plans to improve quality outcomes. Collaborate with physicians, providers, and organizational leadership on quality-of-care initiatives. Supervise, mentor, and support Wellness Registered Nurses and Health Educators. Monitor chronic disease management, preventive care, and behavioral health quality measures. Develop strategies to improve clinical performance, regulatory compliance, and patient outcomes. Support patient education initiatives using motivational interviewing and evidence-based care management practices. Ensure compliance with organizational policies, quality standards, and regulatory requirements. Participate in quality reporting, performance improvement projects, and interdisciplinary team meetings. Qualifications Active New York State Registered Nurse (RN) License required. Bachelor's Degree in Nursing (BSN) preferred. Minimum of three (3) years of Quality Improvement experience required. Minimum of two (2) years of leadership or management experience required. Demonstrated experience with HEDIS and NCQA quality standards required. Previous experience in Ambulatory Care, Community Health, Population Health, Managed Care, or Medical-Surgical Nursing preferred. Strong analytical, leadership, organizational, and problem-solving skills. Excellent verbal and written communication skills.
