Yes What You'll Do Serve as a trusted resource and advocate for patients and families Conduct outreach and community engagement activities Assist patients in navigating healthcare systems and available services Coordinate care between primary care providers, specialty providers, and community organizations Support case management activities and follow-up services Connect patients with critical resources including: Food assistance Housing resources Financial support programs Transportation services Community-based services Educate patients on available healthcare and social service programs Help reduce barriers to care and improve patient access to services Participate in community events and outreach initiatives Maintain professionalism and compassion while serving diverse populations Required Qualifications Passion for serving underserved and low-income communities Desire to work in a community-based healthcare setting Ability to build trust and establish positive relationships with patients Strong communication and interpersonal skills Ability to work independently and collaboratively within a team environment Experience coordinating services and community resources preferred Preferred Qualifications Bilingual Spanish strongly preferred Additional language skills highly valued Previous Community Health Worker, Case Management, Human Services, Social Services, Outreach, Care Coordination, or Patient Navigation experience preferred Epic experience preferred Experience working with diverse cultural and socioeconomic populations preferred Why Become a Community Health Worker? Minimum Requirements:
