POSITION SUMMARY
The Community Health Worker (CHW) is responsible for helping complex clients to navigate and appropriately access community services and resources. The CHW works alongside the treatment team, taking an integrated approach to community outreach and care management. The CHW will provide client advocacy, social support, case management and provide linkage and referral services under the direction of the Team Lead and Exodus Leadership.
MINIMUM QUALIFICATIONS AND EDUCATION
Extensive lived experience and/or professional experience in working with complex, homeless populations who are high users of multiple public health systems. Two (2) years clinical experience in health sciences or related field. Strong verbal and written communication skills. Ability to work independently and show mature judgment. Will be screened for driving competency through the human resources onboarding process.
EDUCATION
High School Graduate or GED equivalent. Valid California driver's license and a good driving record is required. Must be 21 years or older to drive. Peer Advocate or Community Health Worker certification preferred.
