Member Enrollment Job Overview We are seeking enthusiastic and reliable Community Health Workers to join our outreach team. As a trusted, culturally responsive presence in the communities we serve, you'll be the bridge between neighbors and a benefit that can meaningfully improve their health. The primary goal of this role is to drive member enrollment at community events — engaging prospective members, educating them on how medically tailored meals help manage conditions like diabetes and high blood pressure, screening for eligibility, and completing enrollment on-site. Community Health Workers also help create welcoming, memorable experiences for attendees by ensuring events run smoothly and efficiently. This role requires a proactive attitude, excellent communication skills, genuine empathy for the people we serve, and the ability to work well in a fast-paced environment.

Enrollment Quality:
The accuracy and completeness of every enrollment submitted.

Build trust and rapport:
Serve as a trusted, culturally responsive liaison between our organization and the community, meeting people with empathy and respect and reducing barriers that keep them from accessing care.
Handle registration:
Manage attendee registration processes, including enrolling members and distributing program materials and benefit information.
Capture leads:
Collect contact information from interested prospects who don't enroll on-site and pass qualified leads to outreach coordinators for follow-up.
Connect members to resources:
Recognize other needs that come up in conversation and point members toward appropriate community resources and support services.
Document accurately:
Complete enrollment forms and records carefully and submit them through the correct system so no member is lost between the event and follow-up.
Support event execution:
Assist outreach coordinators in executing event plans, schedules, and on-site logistics.
Set up and break down:
Help with event space setup and breakdown, including arranging seating, tables, signage, and decorations.
Deliver customer service:
Provide exceptional service to guests, addressing inquiries and ensuring their needs are met.
Maintain compliance:
Monitor event activities to ensure compliance with CMS, HIPAA, safety regulations, and company policies, and protect member privacy at all times.
Keep the space organized:
Collaborate with team members to maintain a clean and organized environment throughout the event.
