Job Summary:

Kingswood Community Center is hiring a Community Health Worker to support community members with health education, outreach, resources, and service coordination. This role helps connect individuals and families with appropriate health and community services while promoting wellness, accessibility, and positive health outcomes. Common Job Roles Community Health Worker Community Health Outreach Worker Health Services Coordinator Community Wellness Associate Patient Support Representative Key Responsibilities Provide health education and wellness information to community members. Connect individuals with appropriate healthcare and community resources. Assist clients with navigating available health and social services. Conduct community outreach and support engagement activities. Help schedule appointments and coordinate service referrals when appropriate. Maintain accurate and confidential client records. Communicate with healthcare providers, community organizations, and service teams. Identify routine barriers to accessing services and help connect clients with resources. Support community health programs, workshops, and educational events Follow confidentiality, privacy, safety, and organizational procedures. Requirements High school diploma or equivalent required Previous community outreach, healthcare, social services, customer service, or human services experience is preferred. Strong communication and interpersonal skills Excellent organizational and recordkeeping abilities Ability to work respectfully with individuals from diverse backgrounds Basic computer and documentation skills Professional, dependable, and community-focused Preferred Skills Community health or outreach experience Knowledge of local health and social service resources Strong relationship-building and communication abilities Experience with client records or case management systems Good problem-solving and referral coordination skills Ability to work independently and as part of a team About the
