Job Description
CAN Community Health is seeking a Community Health Worker to support underserved communities affected by HIV, hepatitis C, and STIs. In this frontline role, you will provide outreach, education, and client support, helping individuals access testing, treatment, and prevention services. You will connect clients to clinic appointments, social services, and community resources while promoting culturally competent, stigma-free care. Working within a mission-driven, inclusive team, you will collaborate with clinical and support staff, maintain accurate documentation, and participate in community events that advance public and community health.
