Baptist Medical Center is seeking a Community Health Worker to join its care coordination team at its Downtown Jacksonville location. This is a full-time, day-shift opportunity focused on improving patient outcomes and strengthening community health.

Community Health Worker, Job Responsibilities:

Serve as a bridge between healthcare providers, community service organizations, and patients to improve care coordination and access. Build trusted relationships within the community by understanding patients' cultural, social, economic, and language needs. Support patients within a primary care medical home model, ensuring continuity of care. Provide health education and guidance for managing chronic conditions such as Diabetes, Cardiovascular disease, and COPD Help reduce health disparities and improve overall quality of life for the populations served.

Experience Requirements:

1-2 HealthCare experience
