care Clinical confidentiality policies Referral coordination Communication skills Full Job Description Community Rehabilitation Center (CRC). Grant-Funded Position | $20.74-$25.00 per Hour. Community Rehabilitation Center (CRC) is seeking a compassionate Peer Navigator to support individuals living with HIV through Project AIM. The Peer Navigator will provide individualized peer support, system navigation, advocacy, education, emotional support, and linkage to healthcare and community resources. The position will work collaboratively with CRC staff, healthcare providers, and community partners to help participants improve health outcomes and remain connected to care.

Key Responsibilities:

Conduct intake and assessment of program participants. Provide peer education and emotional support related to living with HIV, sexual health, mental health, trauma, and substance use. Assist participants with navigating healthcare and community-based services. Provide referrals and accompaniment to appointments when appropriate to support linkage to HIV medical care. Connect participants to health, wellness, housing, and other supportive resources. Provide harm reduction education and prevention resources based on participant needs. Support participants in developing self-management and wellness goals. Facilitate or co-facilitate peer support groups and educational activities. Build collaborative relationships with community partners. Maintain accurate program documentation, statistics, and required grant reporting. Maintain participant confidentiality and appropriate professional boundaries. Participate in CRC outreach activities, meetings, trainings, and community events.

Qualifications:

Lived experience that supports the ability to provide meaningful peer support to individuals living with HIV. Knowledge and understanding of HIV, harm reduction, and the needs of people living with HIV. Knowledge of community health and social-service resources. Strong listening, communication, and interpersonal skills. Ability to provide compassionate, culturally responsive, and non-judgmental support. Ability to maintain confidentiality and professional boundaries. Experience with peer support, outreach, group facilitation, or community-based services preferred. Knowledge of trauma-informed care, mental health, substance use, and sexual health preferred. Ability to work independently and collaboratively with healthcare and social-service professionals. This position is funded through the Project AIM grant. Employment and continuation of the position are contingent upon the availability of grant funding.
