Key Responsibilities Community Programs & Workshops Promote, coordinate, and facilitate evidence-based programs, educational sessions, and workshops. Provide information and education on health, wellness, prevention, and available community resources. Conduct outreach to individuals, families, and community partners to increase awareness and participation in programs. Assist with community events, health fairs, presentations, and other outreach activities. Case Management & Client Support Conduct client intake, assessments, and needs screenings. Provide ongoing case management and follow-up to help clients achieve their goals. Connect individuals and families with healthcare, social services, housing, food assistance, behavioral health, employment, education, and other community resources. Assist clients in navigating health and social service systems. Maintain accurate and timely client documentation, case notes, referrals, and program records. Follow up with clients to assess progress, address barriers, and ensure appropriate connections to services. Maintain confidentiality and follow organizational policies and applicable privacy requirements. Youth Programs Support the planning and implementation of youth programs, activities, workshops, and educational opportunities. Develop positive, trusting relationships with youth and families. Encourage youth participation, leadership, healthy decision-making, and community engagement. Assist with recreational, educational, and skill-building activities. Communicate with parents, guardians, schools, and community partners when appropriate. Customer Service & Community Engagement Serve as a welcoming and professional point of contact for clients, families, and community members. Respond to questions, concerns, and requests in a timely and respectful manner. Provide culturally and linguistically appropriate customer service in English and Spanish. Build positive relationships with community organizations, healthcare providers, schools, and other partner agencies. Advocate for clients and help reduce barriers to accessing services. Administrative & Program Support Complete required documentation, reports, data entry, and program tracking. Assist with monitoring program outcomes and client participation. Participate in staff meetings, trainings, and professional development activities. Support other nonprofit programs and organizational initiatives as needed. Perform other duties as assigned. Required Qualifications Bachelor's degree in Psychology, Social Work, Human Services, Community Health, education, or a related field preferred. 3-5 years of Experience working with diverse communities, families, youth, or underserved populations. Experience providing workshops / programs preferred. Experience working in a NPO is a plus. Excellent communication skills.
Advanced computer knowledge:
MS Office, Salesforce is a plus. Customer service oriented. Valid driver's license. Willing to commute to all Ser Familia's locations as needed. Fully bilingual in English and Spanish, both spoken and written.
