Job Overview Join our dynamic team as a Community Health Worker, where you will play a vital role in promoting health and wellness within diverse communities. Responsibilities Engage with community members to assess their health needs and provide tailored support and education. Participate as a driver in our transportation program to assure patients have access to their medical appointments. Facilitate access to healthcare services by guiding individuals through enrollment processes for programs like Medicare and Medicaid. Conduct outreach activities such as workshops, public speaking engagements, and health fairs to promote awareness of health resources. Utilize motivational interviewing techniques to encourage behavioral change and foster motivation for healthier living. Collect and record data related to community health trends, service utilization, and individual progress to support program evaluation. Collaborate with healthcare providers, social workers, and local organizations to coordinate care plans for clients with disabilities or chronic conditions. Educate community members on topics including nutrition, addiction counseling, medical terminology, and public health practices. Work closely with our volunteers and community partners to conduct monthly foot clinics. Experience Prior experience in nursing, social work, or public health is highly desirable. Demonstrated ability to work effectively with individuals with disabilities or those facing complex social challenges. Strong communication skills with experience in public speaking or teaching roles. Familiarity with data collection methods and basic understanding of medical terminology. Knowledge of Medicare policies, nutrition education, and addiction counseling principles is a plus. Experience using motivational interviewing techniques to support behavioral change initiatives. Ability to build trust within diverse communities through empathetic listening and culturally sensitive approaches.
