The Community Health Worker is a frontline public health worker that will serve as a liaison, link, intermediary between individuals, community and service systems. This position facilitates access to services and improves the quality and cultural competence of services. The essential work of a community health worker is to provide a prevention approach to build individual and community capacity to reduce social determinants of health. A community health worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities, including outreach, community education, informal counseling, social support, and advocacy.

8:00 am - 4:00 pm Monday -

Friday:

Full-time 35 hours per week Some evenings/weekends, as needed

Pay:

$17.33 - $22.68 per hour, grant funded.

Job Specifications:

High school diploma or equivalent required with less than one-year certificate program or Postsecondary Education-1-yr certificate or a 2-yr associate's degree; and valid driver's license, proof of insurance, and reliable transportation.

Duties and Responsibilities:

Communication Skills:

mediator among individuals, communities and health and social service systems. 2.

Interpersonal and relationship building:

provide culturally appropriate health education and information. 3.

Service coordination and navigation:

care coordination, case management, and system navigation. Coordinate care among individuals, communities and service systems. 4.

Capacity-building:

provide coaching, social support, and conduct outreach. 5.

Advocacy:

advocating for individuals and communities. 6.

Education presentations and facilitation:

building community capacity through prevention education. 7.

Organization Skills:

organize daily schedule to provide prevention services to individuals and communities through health department programming. 8.

Cultural Competencies:

build and strengthen target communities by effectively interacting with people from diverse cultural backgrounds. 9.

Public Health Knowledge:

combine knowledge of public health and culturally relevant experience to deliver comprehensive prevention services to individuals and communities. 10. Understand health systems and basic disease: Assist individuals to navigate health care needs and connect them to resources that best serve their needs. 11.

Behavioral health:

work closely with community linkages to overcome barriers and adopt healthy behaviors. 12.

Field Experience:

learn to assess communities and individuals to effectively communicate the needs to team members to design, implement, and interpret initiatives. 13. Answer and direct incoming calls as needed.
