Job Description

POSITION SUMMARY
The Community Health Worker (CHW) is responsible for helping patients including students (in a school-based setting), individuals, and their families navigate and access community services and other social resources. The CHW supports providers as well as other staff and community members through an integrated approach to care management and community outreach. The CHW will work on activities that will promote, maintain, and improve health outcomes; provide social support; informal counseling, and be an advocate for patients and their families. QUALIFICATIONS
  • Successful completion of a Community Health Worker formal training program
  • Candidate must be fluent in English; Bilingual preferred
  • Experience working in a multicultural setting
  • Basic computer skills a must
  • Understanding of the community served—community connections a plus
  • Communicates effectively using appropriate language and listening skills
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients
  • Self-starter who works well with others, but can also work independently
RESPONSIBILITIES
  • Establish trusting relationships with community members, patients and their families while providing encouragement and access to medical and social support
  • Collects data using the Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) in an effort to better understand and act on patients' social determinants of health
  • Follow-up with patients via phone calls, home visits, and other locations in the community
  • Assist patients with completing applications and registration forms
  • Conduct eligibility determination, enrollment, and follow-up with uninsured patients
  • Help patients set personal goals and attend necessary appointments to meet the patients' needs
  • Provide referrals for services to community agencies as needed
  • Help patients connect with transportation resources and give appointment reminders in special circumstances
  • Assist patients with connections to housing resources and conduct housing intakes for the Balance of State
  • Exhibit excellent working relations with patients, visitors, and staff and effectively communicate the mission of HCC
  • Work cooperatively with other clinical personnel assigned to the same patient
  • Knowledgeable about community resources appropriate for the needs of students/patients/families
  • Plans and executes outreach events to engage community and provide resources to underserved populations
  • Provides consistent communication to the Director of Population Health to evaluate patient/family status, ensuring provided information and clearly reports progress
  • Provides transportation when necessary to patients

Full-time Pay:
$15.00 - $17.00 per hour
