This full-time paid position with the Institute for Human Services, Inc. is located in Corning, NY. The Community Health Worker will outreach rural populations, helping them access health-related services such as prescription medication, vision/dental care, financial assistance, health literacy/insurance, transportation, healthy foods, housing, and self-advocacy. Responsibilities include assessing needs, building trust, and partnering with assigned clients to provide education on community resources, the value of preventive care, screenings, and timely service access; supporting the closure of preventive care measures, offering long-term case management for chronic conditions; and facilitating participation in chronic disease management, care coordination, and health education.
Essential Duties and Responsibilities:
1. Client Outreach, Navigation, and Support a. Conduct and document direct outreach, navigation, and support for assigned clients via in-person visits, text, phone, patient portal, virtual visits, and mail. b. Actively manage assigned clients to connect them with social and health services, close preventive care measures, and improve outcomes. c. Screen assigned individuals and families for service eligibility and manage referrals, warm handoffs, and closed-loop communication. d. Assess individuals for readiness, motivation, and confidence in becoming their own health advocate. e. Monitor assigned client successes and assist with challenges as needed. f. Complete follow-up calls to ensure connection to/or utilization of referred services has occurred. 2. Health Education and Patient Engagement a. Apply cultural competence and a strength-based approach to consumer and provider engagement. b. Use patient-centered interventions to support holistic health and well-being. c. Use key concepts and refer to evidence-based interventions, including motivational interviewing, teach-back, Coaching for Activation, Chronic Disease Self-Management Programs, and the Care Transitions Model. d. Increase client knowledge of community programs, preventive care, health insurance coverage, renewal processes, and health literacy. e. Advise individuals on establishing and using preferred health care provider patient portals as communication and health record management tools. f. Promote primary care, virtual visits, and preventive screenings to support timely access and engagement in care. 3. Documentation, Compliance, and Confidentiality a. Maintain and actively manage timely, accurate documentation, data entry, reporting, and evaluation records for client caseloads, case notes, chase lists, and client registries in accordance with program requirements, using electronic health records and related systems b. Ensure confidentiality of all client information consistent with HIPAA regulations, IHS policies, and primary healthcare site policies. c. Obtain consents as required by law and policy. 4. Team Collaboration and Organizational Support a. Work cross-functionally with systems and teams to effectively lead and direct outreach and management of assigned clients. b. Serve as a member of the IHS team and continuously identify ways to improve and support the work of IHS. c. Identify and initiate effective ways to support, integrate, and apply IHS services to provide the maximum benefit to those served. d. Perform other duties as assigned.
Required Knowledge and Skills:
Possesses excellent verbal and written communication skills, is poverty-informed, and has strong organizational abilities and professionalism. Demonstrates attention to detail and quality of service, adheres to protocols, and can prioritize tasks in a fast-paced, multidisciplinary environment. Skilled in problem-solving, identifying community needs and resources, and knowledgeable in computer systems and software applications. Exhibits initiative, judgment, discretion, and decision-making to achieve program objectives. Works well both independently and as part of a multidisciplinary team, with geographic knowledge of Steuben and Chemung Counties.
Qualifications:
Associate's degree required, bachelor's degree preferred, in health, human services, education, or a related field, with at least four years' experience in case management, project management, data entry, and support.
Additional Requirements:
Work schedule: Monday to Friday, 8:00 am - 5:00 pm (40 hours/week), in person. Additional weekend and evening work may be required for special events. Must be willing to travel, including air travel. A valid NYS driver's license, clean driving record, and operable vehicle are essential for travel to meetings and events.
