COMMUNITY HEALTH WORKER

One full-time opening for Community Health Worker (CHW) for the Perinatal and Infant Community Health Collaborative serving Onondaga County.

is a partner with Healthy Families, the Division of Maternal and Child Health of the Onondaga County Health Department. The CHW will work collaboratively with Public Health Nurses, health care providers, and other community partners to provide health education, navigation, advocacy and supportive guidance to pregnant and post-partum women/individuals and their infants up to 2 years of age. The CHW will assist clients in overcoming barriers to health care and other needed services to support their own health, improve birth outcomes and infant health, and met individual and family goals. The CHW helps connect clients to reproductive health services and other services and resources in the community. Services are provided through outreach, home visits, and individual and group education. Services are non-judgmental and strength-based. CHWs receive initial training and ongoing training and support to provide client-centered, culturally sensitive, trauma-informed services to pregnant individuals, infants and families. A valid driver's license and full time access to a vehicle are required. The ideal candidate will be a peer of the women and individuals to be served, familiar with the Syracuse and Onondaga County community, and have a High School diploma or High School equivalency with relevant work experience. May have an Associates' or Bachelor's degree. We welcome applications from candidates who are proficient in language(s) in addition to English. Starting wage $22.57/hour. Excellent benefits.
