Yes Position Summary The HIV Peer Navigator is a vital member of Guiding Right, Inc.'s New Hope Wellness Center team, serving as a trusted liaison between clients, healthcare providers, and community resources. This role provides culturally responsive, trauma-informed support to individuals living with or newly diagnosed with HIV. The Peer Navigator plays a key role in improving health outcomes by supporting engagement in care, linkage to treatment, medication adherence, and sustained viral suppression. Essential Duties & Responsibilities Peer Support & Engagement Provide client-centered, non-judgmental support to individuals living with HIV. Utilize lived experience appropriately to build trust, reduce stigma, and promote care engagement. Apply motivational interviewing techniques to support behavior change and adherence. Linkage to Care Facilitate rapid linkage to HIV medical care for newly diagnosed clients. Coordinate appointments for HIV treatment, PrEP/PEP services, mental health, and substance use treatment. Accompany clients to appointments when appropriate. Care Navigation & Coordination Collaborate with interdisciplinary care teams to monitor client progress toward viral suppression. Educate clients on lab results (e.g., CD4 count, viral load) and treatment plans. Conduct follow-up with clients who are out of care or at risk of disengagement. Health Education Provide education on HIV treatment, prevention strategies, U=U (Undetectable = Untransmittable), and harm reduction. Support medication adherence through coaching and practical strategies. Resource & Referral Support Connect clients to essential services including housing, transportation, food assistance, and insurance/benefits. Provide guidance on Ryan White program services and eligibility. Documentation & Compliance Maintain accurate, timely documentation in electronic health records (EHR) and required databases. Ensure compliance with HIPAA (45 CFR Parts 160 & 164) , Ryan White program requirements, and agency policies. Safeguard all Protected Health Information (PHI) in accordance with federal and state laws. Community Outreach Participate in outreach and community engagement efforts to identify and re-engage individuals in care. Distribute prevention materials and provide linkage-to-care information. Professional Development Attend required trainings, supervision, and continuing education. Maintain required certifications (if applicable). Required Qualifications High school diploma or equivalent required. Lived experience as a person living with HIV strongly preferred (or demonstrated experience supporting individuals impacted by HIV). Experience working with diverse populations, including

LGBTQ+, BIPOC

communities, individuals experiencing homelessness, and individuals with substance use disorders. Strong communication, interpersonal, and problem-solving skills. Ability to maintain confidentiality and professional boundaries. Valid driver's license, reliable transportation, and proof of insurance. Ability to work flexible hours, including evenings and weekends as needed. Special Requirements Willingness to travel locally within the Oklahoma City area. Ability to perform outreach and attend community-based events. Work Environment & Physical Requirements Work is performed in clinical, office, and community-based settings.
