The Community Health Worker will be a key member of the health care team working to provide comprehensive, coordinated CHW interventions for Main Line Health patients. As a trusted member of their community, the CHW will work on the ground to promote healthy behaviors, build stronger relationships between the community and health and social services, and increase accessibility to those services. This position is eligible for up to $6,000 per year based upon your Full or Part Time status. Join the Team! Position-Specific Benefits include:

Experience:
Minimum 2-years' work or volunteer experience in human services or healthcare.
Education:
High School diploma or equivalent required
Licensure/Cert:
Pennsylvania State Certification for Certified Community Health Workers or obtained within two years of hire
