Job Summary The Community Support Liaison serves as a connection between Concord Christian Center, potential members, referral sources, community organizations, healthcare providers, behavioral-health providers, social-service agencies, and other community partners. The primary purpose of the position is to strengthen CCC's community relationships, develop appropriate referral pathways, connect individuals with available resources, support program growth, and ensure that referral partners receive professional and timely communication. The Community Support Liaison is a non-clinical position unless the employee separately holds credentials authorizing clinical functions. Responsibilities Community Outreach Develop and maintain relationships with community organizations and referral partners. Conduct outreach to behavioral-health providers, medical practices, hospitals, shelters, social-service organizations, treatment programs, schools, community organizations, and other appropriate referral sources. Represent CCC professionally at community meetings, events, networking opportunities, and outreach activities. Referral Development Educate referral partners regarding CCC's available services and basic eligibility criteria. Receive and track potential referrals. Obtain required referral information and route referrals to the appropriate internal staff. Follow up with referral sources regarding the status of referrals when legally permitted. Maintain an organized referral pipeline. Resource Coordination Maintain current information regarding housing, food, transportation, employment, healthcare, benefits, education, legal aid, crisis resources, and other community supports. Share appropriate resources with staff and members. Identify service gaps and new community partnership opportunities. Program Growth Assist leadership in identifying referral trends and potential areas for agency growth. Track referral sources and referral conversion. Identify organizations that may benefit from partnership with CCC. Support outreach for newly developed agency programs. Data and Reporting Maintain accurate outreach and referral records. Track referrals received, referral sources, outcomes, follow-up activities, and partnership development. Submit routine reports to leadership regarding outreach activity and referral trends.
ROLE EXPECTATIONS
The Community Support Liaison is expected to: Represent CCC professionally in the community. Understand the services CCC actually provides and avoid promising services the agency cannot deliver. Protect confidential information. Obtain or share only the information necessary and legally permitted for referral coordination. Respond to referral partners promptly and professionally. Maintain accurate outreach and referral records. Follow through on commitments. Avoid making clinical diagnoses, treatment recommendations, or medical-necessity determinations unless separately credentialed and authorized to do so. Never guarantee admission, authorization, Medicaid eligibility, or payment. Avoid inappropriate financial incentives, referral arrangements, or other practices that may violate Medicaid, fraud-and-abuse, or agency requirements. Work collaboratively with the QP, QM Director, therapists, Peer Support staff, and leadership. Participate in required meetings and training. Maintain current knowledge of CCC programs and community resources. Requirements Proven experience working within hospital environments such as acute care settings or emergency medicine departments. Hospital experience with Level I or Level II trauma centers is highly desirable; ICU or inpatient experience is preferred. Strong background in hospital medicine, behavioral health, social work, or community health coordination. Knowledge of medical terminology, physiology, diagnostic evaluation procedures, and clinical case management practices. Familiarity with electronic health records (EHR/EHR management) systems and HIPAA compliance standards. Certification in Basic Life Support (BLS), CPR; additional certifications such as addiction counseling or mental health counseling are advantageous. Experience working with children, individuals with substance use disorders or disabilities from diverse cultural backgrounds. Excellent client communication skills along with the ability to work effectively with families and multidisciplinary teams. Strong task prioritization skills combined with a proactive attitude toward patient service and community resource coordination.
EDUCATION AND QUALIFICATIONS
Required:
High school diploma or GED. Strong written and verbal communication skills. Strong interpersonal and relationship-building skills. Ability to maintain confidentiality and follow HIPAA/privacy requirements. Basic computer skills and ability to maintain referral/outreach tracking records. Reliable transportation and valid driver's license when community travel is required.
Preferred:
Associate's or bachelor's degree in human services, social work, psychology, public health, communications, marketing, business, community health, or a related field. Previous experience in behavioral health, healthcare, community outreach, referral coordination, social services, case management, business development, or community engagement. Knowledge of local behavioral-health and SDOH resources.
