Tennessee residents only Position Summary We are seeking a Disease Intervention Specialist 2 / RISE (Recovery, Information, Support, and Engagement) Navigator to provide community-based support to individuals impacted by substance use, overdose risk, behavioral health needs, and communicable diseases. The ideal candidate will have experience with community outreach, peer support, substance use recovery, behavioral health, public health, harm reduction, or related social services . The position involves conducting screenings, providing education and support, connecting individuals with community resources, developing partnerships, and maintaining accurate documentation. Responsibilities Conduct substance use disorder screenings using validated tools. Receive referrals and perform mental health and substance use assessments. Provide crisis intervention and solution-focused counseling. Conduct community-based outreach and education. Educate individuals about overdose prevention, HIV, Hepatitis C, and other communicable diseases. Distribute naloxone and overdose prevention materials. Identify individuals who may benefit from healthcare, treatment, recovery, or support services. Build relationships with community organizations and service providers. Coordinate with mental health agencies, treatment providers, harm reduction organizations, and recovery support programs. Connect clients with substance use treatment, mental health services, primary care, recovery support, and other community resources. Provide peer support and ongoing client assistance. Maintain detailed progress notes and referral documentation. Enter client information accurately into applicable systems. Participate in team meetings, case conferences, and required training. Follow applicable contract, state, grant, and agency requirements. Travel within the assigned service area as needed. Perform other related duties as assigned. Required Qualifications 18 years of age or older. High school diploma, GED, or equivalent. Understanding and support of medication-assisted recovery practices. Experience providing peer support or community-based support. Ability to build and maintain relationships with clients and community organizations. Familiarity with recovery support services. Strong active listening and communication skills. Ability to ask supportive, open-ended questions. Ability to maintain professional boundaries. Cultural competency and ability to work with diverse populations. Strong customer service skills. Proficiency with Microsoft Office. Valid driver's license, vehicle, and proof of insurance may be required. Must be a Tennessee resident. Preferred Qualifications Spanish/English bilingual skills. Experience in substance use recovery or behavioral health. Experience in public health or community outreach. Experience with overdose prevention or harm reduction programs. Experience working with community treatment providers and social service organizations. Experience supporting individuals with substance use or behavioral health needs.
