Position Description Under the direction of the Outreach Coordinator, the Community Health Worker (CHW) will work closely with the PR Health team to deliver coordinated public health education and awareness programming. CHWs will connect families and individuals to health and community resources, identify new community partners, and foster beneficial relationships. The role requires proactive outreach and engagement, promotion of PR Health services, and a commitment to community wellness. The ideal candidate is passionate about improving lives and advancing health equity. Key Responsibility AreasSocial Determinants of Health (SDOH) & Referrals

• 15% Utilize SDOH screening tools to assess client needs. Refer clients to appropriate local resources using collaborative platforms. Become proficient in using electronic health record (EHR) systems. Community Outreach
• 50% Collaborate with the Outreach Coordinator to strategize and implement outreach plans. Actively identify and schedule tabling and community events. Meet outreach targets and generate new engagement strategies. Accurately document all interactions in the reporting portal. Understand the health needs of the target population. Collaborate with other departments to enhance outreach. Office Support
• 15% Support SDOH-related client needs. Assist with compiling outreach data for grant and report submissions. Maintain and update the virtual helpdesk resource database. Complete other administrative tasks as assigned. Support of Community Initiatives
• 15% Attend and support community-based organization (CBO) events and initiatives. Identify partnership opportunities with other CBOs. Report community trends and needs to the Outreach Coordinator. Additional Responsibilities
• 5% Maintain confidentiality of all sensitive information.

Perform other duties related to the mission of Project Ricochet as assigned. Availability for occasional evening and weekend work is required. Qualifications Associate's degree or higher in Social Work, Public Health, Sociology, or a related human services field preferred; equivalent experience in human services, cultural arts, or community organizing also considered. Minimum two years of relevant experience preferred. Knowledge of Social Determinants of Health (SDOH) and familiarity with community-based resources. Experience working with economically disadvantaged populations is preferred. Proficiency in Microsoft Office. Competencies Positive, team-oriented, and community-centered attitude. Fluency in Spanish is a plus but not required. Excellent communication skills (written and verbal). Strong organizational skills and attention to detail. Critical thinking and problem-solving skills. Flexibility and adaptability in dynamic environments. Ability to work independently and take initiative. Professional interaction with staff, volunteers, and partners at all levels. Physical Requirements Ability to sit, stand, walk, use hands, reach, talk, and hear. Occasionally required to stoop, bend, lift, or move items up to 25 lbs. Must have visual acuity for close and distance vision, depth perception, and focus adjustment.
