Community Health Worker Union Overview:
ReGenesis Health Care is seeking an enthusiastic full-time Community Health Worker with excellent patient-care skills and a great attitude to join our well established and growing patient needs in Union County. You'll be a great fit if : You have experience Connecting with the community You have experience Coordinating Community Events and Health Education Materials You have experience advocating for community needs and resources by attending community meetings and events. You have experience managing a caseload of patients. You have experience providing health education. You can read, write, and maintain patient records and document using computer systems. You can help patients feel comfortable in addressing their social drivers of health. You can interact positively with a diverse patient population, of all ages, backgrounds, and personalities. Considered You'll Need:
High School Diploma or equivalent education required. Post-secondary degree in related studies (i.e. Public Health, social work, psychology, or related human services field) (Preferred) Community Health Worker Certified (Preferred) The Community Health Worker (CHW) is responsible for helping patients and their families to navigate and access community services, and resources, and adopt healthy behaviors. The CHW supports providers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their families through community events, health education, and internally meeting with patients. CHW provides social support and informal counseling, advocates for individuals and community health needs, and help to complete applications for assistance as needed.
