CHWs deliver home visiting, care coordination, and family support services using the Pathways Community HUB Institute® (PCHI®) Model, an evidence-based approach that helps families overcome barriers, build skills, and achieve meaningful outcomes. Approximately 75% of visits are conducted in participants' homes, with additional community or virtual visits as appropriate. Duties Verifies eligibility and completes all documentation and tools as needed for enrolling the population being served. Conducts home visits and community-based visits with enrolled families. Works with enrolled participants in the community and their homes to identify needs and connects them to community­ based resources and services to remove barriers to care. Completes intervention plan and case related documentation within the time frame of the established policy. Ensures participants schedule and attend appointments with any providers, and assists with transportation to these appointments as needed. Educates enrolled participants on topics relevant to their situation using the PCHI® Model Learning Modules such as low income, housing instability, family conflict, mental health concerns, parenting, and more. Administers required screenings and surveys to enrolled families. Maintains accurate and timely case documentation of Intake1 and any required state systems as applicable. Maintains a full-time active caseload of no greater than 40 low-risk participants and no greater than 30 high-risk participants. Participates in required training, meetings, and community coalitions. Contributes to quarterly program reports. Conducts outreach to community sites to find individuals from the target population eligible for enrollment into the HOPES program. Obtains CHW certification within one year of hire. Performs other duties as assigned. Skills Associate's degree or higher in a health and human services field Have or have the ability to obtain a Community Health Worker certification within one year of hire Two (2) years of direct service experience in a health and human services field Strong interpersonal and communication skills Ability to communicate effectively, function independently, manage time efficiently, organize work flow, and maintain confidentiality Strong documentation and organizational skills General knowledge of community resources

Preferred Qualifications:

Bilingual in English / Spanish Bachelor's degree in a health and human services field Prior home visiting or care coordination experience
