Vistaadhc@yahoo.com Duties Conduct thorough patient assessments, including medical history, dietary habits, and nutritional status using Electronic Health Records (EHR) systems Develop and implement individualized nutrition care plans tailored to specific medical conditions and dietary needs Provide evidence-based nutrition counseling utilizing motivational interviewing techniques to promote behavioral change Manage clinical nutrition support such as tube feeding and other specialized nutrition interventions Collaborate closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes Monitor patient progress through diagnostic evaluation and ongoing nutritional analysis, adjusting plans as necessary Educate patients on nutrition principles related to chronic diseases, weight management, geriatrics, and hospital care settings Supervise dietetic interns or junior staff members while ensuring compliance with regulatory standards and best practices Skills Extensive knowledge of physiology and clinical nutrition principles applicable across diverse medical conditions Proven experience with medical nutrition therapy (MNT), especially in acute care, long-term care, or hospital environments Strong skills in patient communication and client-centered counseling techniques Proficiency in EMR/EHR systems for documentation and care coordination Expertise in managing nutrition support such as tube feeding and specialized dietary plans Demonstrated ability to supervise dietetic staff or interns effectively while fostering professional growth Familiarity with diagnostic evaluation processes related to nutritional deficiencies or metabolic issues Experience working with individuals with specific dietary needs including geriatrics, chronic illnesses, or weight loss programs Knowledge of chronic disease management strategies and long-term care protocols Ability to perform thorough patient assessments and nutritional analysis for optimal intervention planning Join us in delivering exceptional nutritional care that transforms lives!
