Why Choose This Opportunity? $40.00-$46.00/hour Monday-Friday | 8:00 AM-4:30 PM No weekends Meaningful work supporting participant health and wellness Collaborative interdisciplinary team environment Opportunity to develop individualized nutrition plans and provide hands-on education $500 referral bonus when you refer a qualified candidate who is hired Responsibilities Develop and implement individualized Nutritional Plans of Care (POC) and reassess every six months or as clinically indicated. Assess participants' access to food and identify barriers to obtaining adequate nutrition. Coordinate referrals for meal preparation, Meals on Wheels , grocery shopping, and other nutritional resources. Order and recommend liquid nutritional supplements as needed. Provide nutrition education to participants, caregivers, and staff. Conduct sanitation and safety checks of kitchen areas and monitor food service operations. Document nutrition assessments, care plans, interventions, and progress in participant medical records. Participate in interdisciplinary team meetings to develop and implement comprehensive participant care plans. Collaborate with healthcare professionals to promote optimal nutrition, wellness, and quality of life. Qualifications Bachelor's degree in Food Science, Nutrition, Dietetics, or a related field. Registered Dietitian credential through the Commission on Dietetic Registration (CDR) required. Eligible to practice as a Registered Dietitian in California. Knowledge of current medical terminology and healthcare documentation standards. Strong communication, organization, and interpersonal skills. Ability to work effectively as part of an interdisciplinary healthcare team.
