We are seeking a compassionate and skilled Dietician to join our team at Village Green Rehabilitation and Healthcare Center in Bristol, CT. Responsibilities:
Plan, develop, and implement individualized nutritional plans for residents and patients Conduct assessments and monitor the nutritional status of residents and patients Collaborate with the healthcare team to develop and implement care plans Provide educational resources and support to residents, patients, and family members Conduct food demonstrations and nutrition counseling sessions Participate in Quality Improvement initiatives and contribute to the development of policies and procedures Maintain accurate and detailed records of patient care and nutrition plans Stay abreast of industry developments, trends, and best practices in nutrition and dietetics About the

Requirements:
