Job Description

Overview We are seeking a dedicated and knowledgeable Dietitian to join our healthcare team. The ideal candidate will possess a strong background in Dietetics and demonstrate expertise in patient assessment, care planning, and nutrition management. This role is essential in providing high-quality nutritional care to patients, particularly in a home healthcare setting and with a focus on geriatrics. Responsibilities Conduct thorough patient assessment to evaluate individual dietary needs and preferences. Develop and implement personalized care plan2 that address the nutritional requirements of patient. Monitor and adjust dietary plans based on patient progress and changing health conditions. Provide education to patient and their family regarding nutrition, dietary restrictions, and healthy eating habits. Stay updated on the latest research in nutrition and dietetics, applying this knowledge to improve patient outcomes. Maintain accurate records of patient assessments, care plans, and progress notes within healthcare systems. Experience Bachelor's degree in Dietetics or related field; Master's degree preferred. Registered Dietitian Nutritionist (RDN) credential required. 2 years experience in an Adult Daycare setting Previous experience working in a home health setting is highly desirable. Strong knowledge of physiology as it relates to nutrition and health management. Experience in geriatrics is a plus, with an understanding of the unique dietary needs of older adults. Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams. Join us in making a difference through nutrition-focused care!
