Certified Diabetes Care and Education Specialist (CDCES) Registered Dietitian Outpatient services are provided

Responsibilities Patient Care and Education Using evidence-based weight-inclusive practices, conducts assessments, interventions, DSME, and follow-up in a patient-centered manner for patients referred by area providers. Collaborates with a team of dietitians. Develop, conduct, maintain, and provide support for diabetes and nutrition group education programs. Utilize motivational interviewing (MI) skills to support patients in a patient-centered manner. Administrative Participate in collaboration of care and marketing to area providers. Utilize technology effectively and efficiently for scheduling, communications, and record keeping. Maintains accurate records and completes documentation in a timely manner. Provide oversight and record-keeping, compliance for ADCES accredited diabetes program. Qualifications Required RDN credential and State of Oregon Licensed Dietitian CDCES credential Belief in and adherence to Health at Every Size® and Intuitive Eating philosophies

Schedule:

Monday to Friday Some weekends required: Up to 8 Saturdays per year Company Details We are a team of nutritionists, diabetic educators, and massage therapists. Synergy follows a Health At Every Size® philosophy and strives to provide innovative, trauma-informed, and inclusive services for all members of our community. Our team operates in a close-knit environment offering genuine support and relationships. Synergy's diabetes program is accredited by the Association of Diabetes Care and Education Specialists (ADCES) and provides individual and group educational support to patients of all ages. Our diabetes programming and medical nutrition therapy services are offered at 3 locations throughout Central Oregon. Clientele served include people with diabetes of all ages and types, and medical nutrition therapy clients with varied conditions. Services are provided to individuals and groups.
