Dietitian in Hudson Valley, New York Permanent and full-time direct hire Day shift Hybrid Schedule - 2 remote days, 3 in office
Summary:
We are seeking a knowledgeable and compassionate RDN to provide high-quality nutrition therapy and diabetes education. This role focuses on empowering individuals and families to manage diabetes and related chronic conditions through evidence based nutrition care, education, and behavior change strategies.
Responsibilities:
Provide individualized Medical Nutrition Therapy (MNT) for patient with diabetes, prediabetes, and related chronic conditions Deliver comprehensive diabetes self-management education and support (DSMES) in individual and group settings Assess nutritional status, develop care plans, and monitor outcomes in accordance with evidence based guidelines Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient-centered care Educate patients on meal planning, carbohydrate counting, insulin management, glucose monitoring and lifestyle modification Document patient encounters, care plans, and outcomes accurately in the electronic health record Maintain compliance with ADA, ADCES, and organizational standards and protocols Participate in quality improvement initiatives, program development, and community education tasks as needed Stay current with advances in nutrition science, diabetes technology, and clinical guidelines
Requirements:
License Required:
RD/ RDN and CDN Must have a New York State License
CPR/ BLS
Certification Education:
Bachelors Degree Required Masters Degree Preferred Job Type:
Full Time Pay:
