Duties include:

Admissions & Referrals, Evaluate and Accept Appropriate New Residents Admissions, New Admit and Readmit Documentation Checklist Reviews Antibiotic Stewardship Program-Participate, Provide Data and Followup Audits, identify and perform any documentation audits needed. Followup Behaviors Monitor, Documentation, Interventions Care Plan Meetings

• Participate in Interdisciplinary Resident Care Conferences Care Plans
• Contribute to & Update Assigned Care Plan and MDS Sections Communicate Effectively in a Positive Manner
• Residents, Families, Staff and Providers. Dietician/Dietary Recommendations
• Process and Implementation Dining-Supervise and Assist Staff, Ensure Positive Resident Dining Experience. Ensure Resident Appointments, Communicate for Transportation Environmental, ensure unit, equpment, desk, med and storage rooms are in compliance. Fall Program
• Meetings, Notes, Followup, Interventions Infection Prevention/Control Program
• Participate, Provide Data, Document, Followup Investigations-Participate in Administrative Abuse and Neglect Investigation Process Medication Administration
• Supervise/Manage-Provide Inservice Meetings, Department Manager, Weekly, Monthly
• Attend/Participate Nursing Assessments
• Ensure Completed and Accurate Nursing Documentation Reviews Patient Care-Supervise and Assist Staff Perform Rounds (Compliance and Safety) Pharmacy Recommendations
• Process and Implementation Physician's Orders
• Review and Ensure all staff following orders Physicians-Rounds, Communication & Follow Up Providers-Rounds, Communication & Follow Up Psychotropic Monitoring Program
• GDR, tied to Dx and Behaviors Quality Assurance Committee
• Participate and Provide Data Resident Immunization Program
• covid, tb, pneumo, flu, etc Safety Committee, Participate and Provide Data Showers & ADL Tracking Staff Management
• Support, Supervise, Direct, Correct Unit Staff
• New Employee Inservice and Orientation when needed Weight Monitoring and Intervention Program
• Documentation Wound Monitoring and Intervention Program
