Looking for something different? Are you ready to love coming to work? The Cottages at Garden Grove is looking for motivated Assistant Director of Nursing to join a great team in our person centered home environment. The Cottages at Garden Grove is set on a lovely campus in Cicero, NY and is made up of 12 small-home residences. This model of small-homes sets a new standard for skilled nursing and short-term rehab in CNY. These homes come complete with a sense of neighborhood and welcoming atmosphere that will make everyone who resides and works here feel at home.

Must be a graduate of an accredited school of practical nursing and a current licensure with the New York State Board of Nursing Excellent Communication Skills Experience in long term care preferred Essential Duties- Conducts comprehensive chart audits.
- Performs daily clinical rounds to oversee nursing operations, evaluate resident care, identify areas requiring intervention, and ensure timely follow-up and resolution of clinical concerns.
- Provide leadership and oversight of nursing staffing operations to ensure appropriate staffing levels based on resident acuity and needs.
- Assist in maintaining accident/incident report log and files. Responsible for ensuring follow up is complete by Supervisors in a timely and thorough fashion. Assist in maintaining communication of events with appropriate contracted Agencies and inform DON and administrator. Trending completed.
- Ability to complete though investigations in conjunction with the DON / Administrator and conclude when an incident qualifies as a NYS DOH reportable incident and complete the NH Incident Form via the HCS website.
- Assists DON/ Administrator during NYS surveys and in the process of preparing, submitting and instituting a plan of correction.
- Assist DON with writing, reviewing Nursing Policies and Procedures with ensuring all staff are knowledge of policies and procedures and assist with completing staff in-services / competencies
- Assist DON with proper collection / storage and submitting approval for Narcotic destruction quarterly in compliance with the DEA and DOH regulations.
- Attend morning team meeting as an opportunity to be aware of Complaints, Accident/incidents, resident status, and general organization operations.
- Pulls reports from Team TSI and conducts necessary analysis for quality improvement
- Oversee and maintains accurate par levels for each individual cottage and adjusts those par levels based on trends and communication with individual cottage staff. Ensure that the weekly orders are accurate and completed.
- Assists with disciplines to staffing, including writing and issuing
- Oversee the employee mentor program and ensure that new hires receive proper training and skills
- Completes monthly employee annual physicals
- Screens/interviews candidates for nursing positions
- Acts as a backup in the absence of a Nurse Manager and Director of Nursing
