•Assistant Director of Nursing - Transitional Care Unit

• to join our compassionate healthcare team. Responsible for clinical leadership, care planning, and family communication. Ensure completion of care plans, coordinate physician visits, and manage wound care. Provide coaching to staff, handle discharge planning, and oversee certifications. Participate in resident meetings, act as therapy liaison, and perform reassessments. Assist in survey management, doctor rounds, and serve as on-call nurse. Qualifications include a valid RN license, at least 3 years' long-term care experience, and computer skills. Must demonstrate attention to detail, regulatory knowledge, and resident-centered care principles.
