Our model leverages advanced technology and proactive care strategies to reduce hospitalizations, improve quality of care, and support patients in long-term care settings. Position Overview We are seeking a Registered Nurse (RN) to serve as a Regional Director, overseeing clinical operations across multiple Skilled Nursing Facilities (SNFs) throughout California & Nevada. This is a non-bedside leadership role focused on: Team management Operational oversight Facility relationships Patient care coordination Key Responsibilities Manage and oversee a team of field technicians working within SNFs Coordinate and monitor new patient admissions and discharges, ensuring timely onboarding into services Ensure all eligible patients are scheduled and seen appropriately by field staff Conduct monthly on-site visits to partner facilities to meet with administrators and maintain strong relationships Review and oversee care plans developed by nursing staff to ensure quality and compliance Monitor workflow efficiency and ensure services (RPM, CCM, telehealth) are being delivered consistently Serve as the primary point of contact between facilities and internal clinical/operations teams Identify operational gaps and implement process improvements Qualifications Active Registered Nurse (RN) license in California & Nevada 3+ years of nursing experience (SNF, care management, or leadership experience strongly preferred) Strong organizational and leadership skills Experience coordinating care across multiple patients or sites Comfortable working with technology and clinical platforms Excellent communication skills with both clinical teams and facility leadership Ability to travel locally throughout Florida Please note: This is a hybrid role that requires occasional monthly travel to various skilled nursing facilities (SNFs) and regional care centers.
