Job Overview Seniors Helping Seniors (SHS) Home Health is seeking an experienced and compassionate Assistant Director of Nursing (A-DoN) to support and oversee the delivery of high-quality home health services. This role is critical to ensuring clinical excellence, regulatory compliance, and coordinated patient-centered care for clients receiving skilled nursing, occupational therapy, physical therapy, and speech therapy services in their homes. The Assistant Director of Nursing provides clinical oversight, guidance, and support to field staff while collaborating closely with the therapists, physicians, and caregivers. The ideal candidate demonstrates strong clinical judgment, leadership ability, and a commitment to promoting safe, effective, and dignified care for seniors. This position plays a key role in maintaining compliance with state and federal regulations, agency policies, and professional standards while fostering a culture of quality, accountability, and compassionate service.
Qualifications:
  • Graduate of an accredited school of professional nursing; BSN.
  • Current license as a Registered Nurse in the state of Texas.
  • Minimum of two (2) years nursing experience in a home health setting or equivalent experience with one year of supervisory experience.
  • Complies with accepted professional standards and practice.
  • Proven experience using CRM software, WellSky/Kinnser, to document patient care.
  • Proven experience conducting and documenting comprehensive OASIS assessments (Start of Care, Recertification, Transfer, Discharge, etc.) to ensure compliance with Medicare Conditions of Participation.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Knowledge of home care federal and state regulations.
  • Knowledge of reimbursement sources and documentation requirements within home health care.
  • Must be a licensed driver with automobile insured in accordance with state requirements.
  • Has current CPR certification, HEP B vaccination, and TB testing documentation.
Essential Functions/Areas of Accountability:
1. Manages and directs a team of RNs, LVNs and therapists to ensure safe, effective, and appropriate home care services. 2. Is always available during regular business hours, in person or by telephone, and as needed to provide support and assure quality care delivery to home care clients. 3. Receives referrals, determines services required, and SHS Home Health Agency's ability to meet needs. Assigns appropriate clinicians to cases. 4. Meets with Case Managers/clinical staff on a regular basis to provide guidance and information related to specific issues. Provides directions to teams to ensure that client needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals. 5. Attends case conferences and other clinical meetings to facilitate coordination of care. 6. Collaborates with Director of Nursing and Administrators in establishing operating budget needs and priorities for the department. Reviews monthly team member's productivity information. 7. Ensures that a client's plan of care or care plan is executed as written. 8. Provides on-call backup for Case Managers, as needed. Develop working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to clients. 9. Provides support and direction to SHS Home Health Agency staff, other health care professionals, clients, and families related to appropriate and available health care resources. 10. Educates SHS Home Health Agency staff on clinical services, policies, and procedures, as needed. 11. Facilitates problem-solving sessions to enable Case Managers and other SHS staff to resolve client and/or reimbursement source issues. 12. Stays current on available community resources, health care costs, and industry trends through self-education and access to outside educational opportunities to meet 12 CEU credits yearly. 13. Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with SHS Home Health Agency policies and procedures, regulatory requirements, and industry standards. 14. Monitors open and closed charts regularly and participate in the quarterly clinical record reviews. 15. Reviews of SHS Home Health Agency policies and procedures, recommend changes or revisions as needed and attend committee meetings. 16. Provides educational programs and information regarding appropriate documentation practices. 17. Provides leadership to team and support staff in identifying SHS Home Health Agency/client needs and opportunities for quality improvement. 18. Assists with marketing, public relations, and discharge planning by participating in departmental meetings. 19. Assists quality improvement teams with data collection for the Quality Improvement Plan established by the SHS Home Health Agency. 20. Reviews OASIS data collection to assure accuracy and consistency with requirements. Communicates with clinicians and provides training as indicated to achieve compliance with collection and reporting timelines. 21. Ensures that an appropriate health care professional performs a reassessment of client's needs when: (1) there is a significant health status change in the client's condition, (2) at the physician's request; or (3) after hospital discharge. 22. Identifies and implements changes in clinical and/or operational practice based on the findings of the Quality Improvement Program. 23. Interprets and enforces human resource policies and procedures in a fair and consistent manner. 24. Assists in the screening and interviewing of new SHS Home Health Agency personnel and makes recommendations to Director. Assists in the orientation of new personnel. 25. Conducts timely performance evaluations in accordance with SHS Home Health Agency policy. 26. Provides on-site supervision per SHS Home Health Agency policy and as needed to determine staff competency and respond to educational and developmental needs. 27. Follows SHS Home Health Agency guidelines for disciplinary actions. Documents all disciplinary actions in accordance with Human Resource policies and legal guidelines. 28. Collaborates with the Director of Nursing, Administrators and other clinical management staff to ensure proper staffing of qualified, competent personnel. 30. Promotes personal safety and a safe environment for clients and co-workers. 31. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements. 32. Assesses safety of environment and takes the initiative to help prevent accidents and promote safety. 33. Recognizes and responds appropriately to potentially unsafe situations. 34. Performs other duties as assigned.
