Admissions Director - Skilled Nursing & Rehabilitation Wellsential Health is seeking an experienced, compassionate, and community-driven Admissions Director to lead the admissions efforts for our skilled nursing and long-term care facility, located in La Grange, TX. This role blends community outreach, clinical understanding, and relationship-building with the goal of ensuring a seamless, supportive admissions experience for residents and families. You'll serve as the face of our care centers, hosting facility tours, attending events, and coordinating care transitions with hospitals and referral sources.

Key Responsibilities:

Oversee and support admissions processes Build strong referral networks with hospitals, discharge planners, social workers, and physicians Attend and host local community events to promote services (short-term rehab, long-term care, and therapy) Collaborate with clinical teams to ensure potential residents' medical and care needs can be met Coordinate tours and intake assessments with families and responsible parties Ensure all admissions documentation is accurate, timely, and compliant with state/federal standards Monitor admissions metrics and drive census growth across assigned locations

What We're Looking For:
