Skilled Nursing Facility (SNF) / Short-Term Rehabilitation Position Summary Shoreline Care Center is seeking an experienced and motivated Assistant Director of Rehabilitation (ADOR) to support the daily operations of our rehabilitation department in a skilled nursing and post-acute care setting. The ADOR will work closely with the Director of Rehabilitation to ensure the delivery of high-quality therapy services, regulatory compliance, clinical excellence, and positive patient outcomes. The ideal candidate must possess an active California Physical Therapist (PT) license or Occupational Therapist Registered (OTR) license , along with prior experience in a Skilled Nursing Facility (SNF) or post-acute rehabilitation environment.

QualificationsRequired:

Active California Physical Therapist (PT) or Occupational Therapist Registered (OTR) license. Minimum of 2 years of Skilled Nursing Facility (SNF) or post-acute rehabilitation experience . Strong knowledge of Medicare, Managed Care, and rehabilitation regulations. Excellent leadership, communication, and organizational skills. Ability to collaborate effectively with interdisciplinary teams.

Preferred:

Previous supervisory or management experience in rehabilitation services. Experience with rehabilitation documentation systems and rehab management software. Essential Duties and ResponsibilitiesClinical Leadership & Program Support Assist the Director of Rehabilitation in overseeing daily rehabilitation operations. Support Physical, Occupational, and Speech Therapy services to ensure quality patient care. Monitor patient outcomes and assist in implementing performance improvement initiatives. Promote evidence-based clinical practices and rehabilitation excellence. Department Operations Assist with scheduling, staffing coordination, and productivity management for rehabilitation staff. Help maintain department efficiency while ensuring quality care standards are met. Monitor therapist caseloads and workload distribution. Participate in budgetary and operational planning when requested. Regulatory Compliance Ensure compliance with federal, state, Medicare, Medi-Cal, and facility regulations. Support compliance with documentation standards and reimbursement requirements. Assist with preparation for surveys, audits, and regulatory inspections. Monitor therapy documentation for accuracy, completeness, and timeliness. Staff Development Assist with onboarding, training, and mentoring rehabilitation staff. Provide support and guidance to therapists and therapy assistants. Promote a positive, collaborative, and patient-centered work environment. Participate in performance evaluations and staff development initiatives. Interdisciplinary Collaboration Work closely with nursing, social services, dietary, case management, and medical staff. Participate in care plan meetings and interdisciplinary team discussions. Communicate patient progress and discharge recommendations to appropriate team members. Support safe transitions of care and discharge planning. Patient Care Maintain a clinical caseload as needed. Evaluate and treat patients within the scope of PT or OT licensure. Develop individualized treatment plans designed to maximize patient independence and functional outcomes. Ensure adherence to ethical and professional practice standards. Physical Requirements Ability to stand, walk, bend, lift, and assist patients throughout the workday. Ability to lift up to 50 pounds with or without reasonable accommodation. Ability to perform all essential functions of a licensed therapist in a rehabilitation setting.
