Responsibilities Lead and manage on-site rehab services within skilled nursing and post-acute facilities Oversee daily clinical operations, staffing, scheduling, and caseload management Ensure high-quality, resident-centered PT, OT, and ST services and measurable outcomes Support, mentor, and develop therapy staff; foster collaborative, clinician-centered culture Monitor documentation, billing practices, and compliance with all regulations and facility policies Collaborate with facility leadership and interdisciplinary teams to align rehab goals with resident needs Track and report quality, functional outcomes, and performance metrics to leadership Drive continuous improvement initiatives and evidence-based clinical practices Engage in family/resident communication regarding rehab plans and progress Support recruitment, onboarding, and retention of therapy team members Required Skills Rehabilitation program management Clinical operations management Skilled nursing/post-acute rehab Physical/occupational/speech therapy coordination Regulatory and compliance knowledge Interdisciplinary team leadership Caseload and productivity management EMR and clinical documentation Quality improvement and outcomes tracking Staff coaching, mentoring, and training
