2997543787-2 Apply for this job now Job Description Baptist Health Rehabilitation Hospital seeks a Case Management Director to lead our case management team in delivering coordinated, patient-centered rehabilitation care. This role oversees discharge planning, utilization review, and care transitions for patients recovering from complex conditions such as stroke, brain injury, and spinal cord injury. The director will mentor case managers, collaborate with physicians and interdisciplinary teams, ensure regulatory compliance, and drive process improvements that enhance outcomes, patient experience, and length-of-stay efficiency in a faith-based, mission-driven environment. Responsibilities Lead and manage the inpatient case management team Oversee discharge planning and safe care transitions Coordinate interdisciplinary care with physicians and rehab teams Monitor utilization review and length-of-stay efficiency Ensure compliance with regulatory and payer requirements Develop and implement process improvements and best practices Mentor and train case managers and social workers Collaborate with leadership on patient flow and capacity planning Track and report case management and outcome metrics Support faith-based, patient-centered culture and service excellence Required Skills Case management in acute or inpatient rehab Discharge planning and care transitions Utilization review and length-of-stay management Interdisciplinary care coordination Knowledge of CMS and payer regulations Staff leadership and team development Data analysis and outcome tracking Electronic medical record (EMR) systems Conflict resolution and negotiation Process improvement and quality initiatives
