The Rehab Liaison :

Assesses patients at Reading Hospital for potential placement at the Reading Rehab Hospital's Acute and Subacute Transitional Care units. Coordinates admissions and transfers in accordance with established standards, guidelines, and admission criteria. Collaborates with physicians, department leadership, and case management to prioritize admissions and determine the most appropriate available bed for each patient.. Evaluates patients' medical and functional needs to determine appropriate placement based on unit-specific admission criteria. Communicates and collaborates with hospital departments to streamline processes, maximize efficiency, and expedite admissions and transfers. Promotes Reading Rehab Hospital's services and serves as a key liaison between hospital departments, patients, physicians, referral sources, and current and prospective clients. Provides excellent customer service to physicians, patients, office administrators and staff, as well as hospital clients, including nursing homes and other healthcare facilities. Builds and maintains strong, collaborative relationships with hospital departments, committees, medical staff, referral sources, and community partners. Primary work location will be at the

• 1-2 days per week (8am-4:30pm) primarily on Wednesdays and Fridays. Additional hours possible as schedule permits. FT

• OR•combination of relevant education & experience may be considered in lieu of Vocational/Technical Degree Experience Experience in Acute Rehab or Skilled Nursing is preferred.

Experience with electronic medical records preferred. Liaison experience preferred but not required.
